Your hair grows about six inches each year. Your color history stays with your hair until it has been cut off. Based on how long your hair is, please list all color services that you've received for that time frame.

 

If 'At home color' applies, please limit your service selections to Help Me Rhonda as we will need to complete a strand test prior to booking any other services. I respect both your time and wallet and would not want to be inconsiderate of that if achieving your goals is not possible.

Deposits and Cancellation Policy

Deposits:

When scheduling color services requiring longer than 2 hours of my time, there will be a non-refundable $100 booking deposit made to reserve your appointment and that amount will go toward your final service balance. That amount may also go up if the service(s) require a significant portion of my day.

 

If needing to reschedule, any deposits made will transfer as long as you reschedule with a minimum of 48 hours notice from the time of your appointment. 

 

Cancellations: 

If needing to cancel or reschedule your appointment, a minimum of 48 hours notice is required to avoid any cancellation penalties. 

 

Rescheduling, cancelling your appointment within 48 hours, failure to show for your appointment, or arriving late to the point where I need to terminate your appointment are all considered last minute cancellations. Any last minute cancellations will result in losing any existing deposits made and will require submitting an additional deposit prior to scheduling your next service. Last minute cancellations for services without a deposit will result in a penalty fee in the amount of 50% of the missed service(s) and will be due prior to scheduling any future appointments. If last minute cancellations continue or occur during your first service, including consultation, with me, I do reserve the right to refuse future services all together.

I will reach out to you soon either via email or phone call with some time slots to set up your appointment. Following that I'll send you confirmation with a welcome email giving you some helpful information including address and parking. You may also receive a phone call if your service requires a deposit or if there are any questions I have regarding this form. Please have any inspirational hair photos saved in a separate album on your phone for easy access during our consultation. No personal inspiration photos please. Masks are required.

By submitting this form you understand that this is the first step in my overall color process. You also acknowledge that I may require a deposit depending upon which service you selected and agree to the terms stated above regarding the deposit.
